Below you will see a link that will take you directly to our vaccination appointment pages ONLY for COVID-19 there.
After you get an appointment it is very important that 24-48 hours prior to your appointment you fill out the COVID vaccination consent form. Also if you have not filled a prescription at Lopez Island Pharmacy in the past year you should also fill out the new patient intake form.
Having all the paper work done ahead of your appointment will greatly speed up getting you in and out of the facility, and minimizes social contact.
Also when you arrive for your appointment be sure to bring and wear your mask.
